Each Elevate 920 member gets THREE nominations per quarter. You can nominate three different charities one time, or you can nominate one charity three times (be sure to enter the charity on all three lines). If you have any additional contact information for the charity, enter that as well.
CHARITY REQUIREMENTS: The nominated charities must be a registered 501(c)3 in good standing and they must do work in the greater Fox Cities.
